Waiver and TAC Application Updates

The TAC Waivers and Membership Applications have been updated.
Please read the following carefully;

In an effort to ensure that we have complete records, the TAC board has requested that all participants print out and submit a new copy of one of the above forms. You will notice that waivers now request the additional information of an emergency contact. This is to ensure that we have someone we can reach for ALL players, in the unfortunate event of any severe injuries or illnesses. Also added to the waivers are a line for a printed name as well as a forum name. We ask that everyone make an effort to please fill out these forms legibly.

In addition, we have added an age verification section at the bottom of both forms. This is to ensure that no players are misrepresenting their age and creating undue liability for the TAC organization. It is important that players who are over the age of 18 present an ID with a listed birthdate to a TAC board member when they turn in their new waivers or membership applications. No forms will be accepted without either a valid ID OR a parental signature!

Please make an effort to bring these new forms out to the next event you participate in, and return them to PhaedrusZen or any TAC board member. We would also request that TAC forum users make an effort to update their profile information to reflect their proper age. Thanks in advance for your cooperation!

(In a very few instances, we have complete records for existing TAC members. Those members will be contacted shortly and informed whether they need an ID for age verification, as papers with an existing parental signatures are still valid. ALL players who only have waivers will need to turn in a new form. If you are not contacted specifically, assume that you will need to fill out new paperwork to continue participating in TAC events.)
