Application for organization enrollment and membership

Completion of this form does not necessarily guarantee membership into the Tucson Airsoft Coalition, known hereafter as TAC.
This form must be completed in full in order for you to be considered for membership in the organization. You are guaranteed
your right to privacy and your information will be used only for TAC purposes. Your information will not be sold or given to any
company or organizations outside of TAC. TAC furthermore guarantees that this information will be used sole by TAC authorized
personnel only and may not be viewed by anyone not authorized by TAC and yourself to view the material contained herein.

Additonally, on October 1st 2007 and forward TAC members are required to pay a $12 annual due. This fee is pro-rated for January payment so if you decide to jon in July you would owe $6 for the remiander of the year like wise if you pay in October the balance would be $3. You also have the option to pay $40 for a life time membership. Dues can be handed in to the TAC secretary or treasurer or paypaled to the following address:

I do hereby affirm by placing my signature on this form that I will abide by the TAC rules, standards and code of conduct
and I understand that I may be relieved of my membership to TAC if I should violate any of the above or if I show any
conduct that is counter productive to Airsoft in general. I further agree to be honorable in all Airsoft games and swear to
abide by the laws ofmy state concerning the use, display, possession, and transportation of any and all Airsoft weapons
and equipment. I further attest that I am signing this application and accompanying liabaility waiver of my own free will
and understand that this applicationdoes not give me membership to TAC until it is approved by the Commanding Officer
of the TAC or other presiding Board Officer. I also understand that if I am under the age of eighteen (18) that my parents
must also sign this application to confirm that they are giving approval for me to join Tucson Airsoft Coalition and to
participate in any and all TAC sponsored events.

LIABILITY WAIVER
Attention! Read the following before accepting!
ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY, AND RELEASE FOR TAC PARTICIPATION

In consideration of me being allowed to participate in any airsoft competition conducted under the auspices of Tucson Airsoft Coalition,
known hereafter as TAC, its club members, organizations and individuals (the Activity), I agree that:

1. I understand the dangers that may be caused by my own actions or inactions, the actions or inactions of others participating
in the Activity and the conditions under which the Activity is conducted. I understand the nature of the Activity and
acknowledge that I am qualified to participate in such Activity. I further acknowledge that I am aware that the Activity will be
conducted in facilities open to the public during the Activity. I further agree and warrant that, if at any time, I believe
conditions to be unsafe; I will immediately discontinue further participation in the Activity.

2. I FULLY UNDERSTAND that: (a) the Activity involves risks and dangers of SERIOUS BODILY INJURY, INCLUDING
PERMANENT DISABILITY, PARALYSIS AND DEATH ("Risks"); (b) there may be other risks and social and economic
losses either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT ALL SUCH RISKS AND ALL
RESPONSIBILITY FOR LOSSES, COSTS AND DAMAGES incurred as a result of my participation in the Activity.

3. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE AND AGREE TO HOLD HARMLESS TAC,
its respective administrators, members, directors, agents, officers, volunteers and employees, local organizing committees,
other participants, any sponsors, advertisers, if applicable, owners, and lesser of premises on which the Activity takes place
(each considered one of the Releases herein) from all liability, claims, demands, losses, or damages on account caused or
alleged in whole or in part by any act or omission of the Releases in connection with the Activity or otherwise, including
rescue operations, and further agree that if, despite this release, I or anyone on my behalf makes a claim against any of the
Releases named above, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY
LITIGATION EXPENSES, ATTORNEYS FEES, LOSS LIABILITY, DAMAGE OR COST ANY MAY INCUR AS THE
RESULT OF SUCH CLAIM.

4. I authorize TAC appointed organizers to act on my behalf if medical treatment is necessary. In the event of illness or injury to
myself (or child if signing for a child), I authorize TAC to obtain medical treatment for myself at the expense of the Responsible Party
signed below below.

5. I agree to abide by all TAC rules and regulations, including to be bound by the arbitration procedures therein, that I am aware of
and understand, for any dispute regarding my right to participate in the Activity, as set forth in the By laws of TAC, as they are
amended on a periodic basis, which I understand are available on the TAC website.

6. I affirm that I am not suspended or banned from play or participation by any club local, foreign or national.

7. I am aware that TAC has the right to revoke my TAC enrollment or participation privileges in any and all future events, in the
event of any violation of the aforementioned statement.

I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP
SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY WITHOUT ANY INDUCEMENT OR
ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL
LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS
AGREEMENT IS HELD TO BE INVALID THAT THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN
FULL FORCE AND EFFECT.
